To notify the Network of a permanent or temporary closure / interruption of service complete the applicable Form found here:
Step 1. Navigate to the IPRO ESRD Website Home Page
Step 2. Click the banner at the top of the screen titled “Facility Closure Reporting”.
Step 3. User will be directed to the Emergency Reporting landing page.

To complete an Emergency Incident Report
Step 1. Please use the link titled “Emergency Incident Reports” if you are reporting any altered schedules or clinic closures that are not being reported in EQRS, including but not limited to:
- Staffing shortages
- Supply chain disruptions
- Facility or infrastructure damage
- Natural disasters
- Cybersecurity events
Step 2. You will be redirected to the Emergency Incident Report and asked to select your CCN# or search by DBA name.
Step 3. Complete with relevant information, including facility operational status, reason for change in operations, event timeline, patient/staff safety, and identification of unmet needs.
Step 4: Once finished, select “Next” to submit:
To Report a Permanent and Temporary Non-Emergency Closures:
Step 1. Please use the link titled “Permanent and Non-Emergency Closures” to document any permanent or temporary closures. This includes but is not limited to:
• Permanent closures
• Long-term construction
• Clinic relocating
• Closure for over 30 days
Step 2. You will be redirected to the Permanent and Non-Emergency Closure Report and asked to select your CCN# or search by DBA name.
Step 3. Complete with relevant closure information, including closure date, reason for closure, patient placement status, and whether assistance from the Network is needed.
Step 4. Once finished, select “Next” to submit:
