Log into the IPRO ESRD Facility Information Management System to view, download, and print the Network / Facility Agreement of Participation on file for your facility.

Password: Facility 6 digit CCN number

Review for accuracy that the Authorized Facility Representative is current, and make any necessary changes.

Click Select All --> Edit --> Save

Keep a hard copy of the Network / Facility Agreement of Participation on file for your next State Survey visit.
