Facilities CAN update: 

• GFR Method Used (click on Default Preferences) 
• Hours/Days Open
• Backup facility information
• To update facility information in EQRS:
  • Log into EQRS https://eqrs.cms.gov/globalapp/
  • Go to Facilities Tab and select Search Facilities
  • Search by Name or CCN

Facilities CANNOT update: 

• Certified Services
• Address
• Phone/Fax
• Ownership
• SUBMIT a document [on CMS or Corporate letterhead] that includes the correct information and Date of Change and the Network will update EQRS for your facility: https://help.esrd.ipro.org/support/tickets/new.
  

EQRS Facility Information is important to maintain so that the Network can correctly assign QIA Goals and IPRO Learn activities. According to the CMS conditions for coverage, facilities are required to notify the state survey agency of the move.
