Instructions for Accessing the Dialysis Facility Reports (DFR)
DialysisData.org web site is used to perform all three of the following activities:

Updating Master Account Holder (MAH) information (use facility CCN as username and MAH password for log in)

Add/edit users and grant permissions (use facility CCN as username and MAH password for log in)

Access DFR and QDFC reports (use individual user email address as username and password setup by them for log in)
To verify/edit the facility’s MAH contact information, the MAH should do the following:
· Go to www.dialysisdata.org
· Enter Username. Username is the facility’s 6-digit CCN number. See figure 1.
· Enter MAH Password provided to you by the Network. See figure 1.
· Click the Log in button. See figure 1.

After logging in, verify current MAH contact information. Click Edit MAH Contact Info and update by entering the MAH name and email address. See figure 2.
