1. Log into EQRS with your HARP Account.

If you do not have the QIP option: apply for access following these instructions: https://esrd.ipro.help/hc/en-us/articles/33114577815315-HARP-EQRS-QIP-Access-Dialysis-Facilities

2. Click on 'Change Organization' to switch from EQRS to QIP

3. Select your facility

4. Select QIP Scores in the top Left

View/Download Reports to access your QIP Preview Score and Patient List of patients being counted in each measure.
