Only fill out [Type of Dialysis Training], [Dialysis Training Start Date], [Dialysis Training End Date] if the patient TRAINED in your facility.

If no Training was provided, leave these fields BLANK.

If a patient requires training for a Home Modality, Add the Treatment Start Date, Add Training information to that Treatment Start Date.

EXAMPLE: The below example has inaccurate information that needs to be fixed:

1. Patient receiving Hemodialysis (treatment) at a Dialysis Facility/Center (setting) on 12/02/2021 does NOT need Dialysis Training information (outlined in green, below)

   -Facility should EDIT & remove [Type of Dialysis Training, Dialysis Training Start Date, Dialysis Training End Date]

2. The Home CCPD admission on 01/01/2021 DOES need to enter: [Training Type, Training Start Date, Training End Date].
