Log into EQRS: https://eqrs.cms.gov/

Click on Dashboard on the upper Left

Monitor and resolve any items listed - especially any 2728 & 2746 Forms that are Due - so that you stay in Compliance!

NOTE: Sometimes the Dashboard will have a glitch and the data will not be current.
